Services
Peak Performance Hydration LLC ("PPH") provides IV therapy and wellness services at 42201 N 41st Dr Ste 122, Phoenix AZ 85086 under the medical supervision of our Board Certified Emergency Physician. All services are performed by licensed medical professionals.
Medical Disclaimer
IV therapy services are not intended to diagnose, treat, cure, or prevent any disease. All treatments are elective wellness services. You agree to disclose all relevant medical history, medications, and allergies before receiving any treatment. PPH reserves the right to decline service if deemed medically inappropriate.
Appointments & Cancellations
Online bookings are confirmed upon receipt of your booking request. We request 24-hour notice for cancellations. Late cancellations or no-shows may result in a cancellation fee. Walk-ins are welcome subject to availability.
Memberships
Monthly memberships are billed on a recurring basis. Unused drips roll over for up to 30 days. Memberships may be cancelled with 30 days written notice. No refunds are issued for partial months.
Liability
By receiving services at PPH, you acknowledge the inherent risks of IV therapy and release PPH, its owners, employees, and medical director from liability for any adverse reactions disclosed in your intake form as known risks.
Governing Law
These terms are governed by the laws of the State of Arizona. Any disputes shall be resolved in Maricopa County, Arizona.
