What I'm consenting to

I authorize Peak Performance Hydration and its licensed clinical staff to administer intravenous (IV) hydration, vitamin/nutrient therapy, and/or intramuscular injections as reviewed and approved by the supervising physician.

Risks I understand

• Common, minor: bruising, redness, swelling or tenderness at the IV/injection site; a cool sensation or vitamin taste during infusion.
• Uncommon: infiltration, phlebitis, lightheadedness, or allergic reaction.
• I will tell staff immediately if I feel unwell during or after treatment.

My acknowledgements

• The information I provided on my intake is accurate to the best of my knowledge.
• IV therapy is a wellness service and is not a substitute for diagnosis or treatment of a medical condition.
• I have had the opportunity to ask questions and they were answered to my satisfaction.
